Job summary

Enhance Rehabilitation is seeking qualified Physical Therapists to join a growing team in a skilled nursing facility (SNF) setting in Naperville, IL. You will evaluate and treat patients following an individualized plan of care, while collaborating with facility staff to ensure high-quality care.

You will supervise Physical Therapist Assistants, document progress and outcomes, and communicate with patients and families about injury, progress and expected outcomes.

Qualifications

• Evaluate and treat patients following an individualized plan of care.
• Supervise Physical Therapist Assistants.
• Complete timely and accurate documentation including evaluations, re-evaluations, discharges and daily progress notes.

• Providing excellent care for our patients
• Providing excellent customer service to our patients and family members
• Collaborate with facility staff to ensure excellent care for your patients
• Evaluating new patients, developing plans of care and setting measurable, functional goals
• Treating patients, answering their concerns and educating them on their injury, progress and probable outcomes
• Supervising Physical Therapist Assistants
• Re-evaluating patient progress, revising plans of care and resetting goals
• Complete timely and accurate documentation including; evaluations, re-evaluations discharges and daily progress notes.
