Responsibilities

• Performs initial patient evaluation and assesses data to identify problems.
• Establishes and documents a plan of care with SMART goals based on evaluation.
• Provides or delegates and supervises physical therapy interventions safely.
• Educates patient and family regarding condition and home program/discharge instructions.
• Performs re-examination, updates plan of care based on progress.
• Communicates relevant patient information with other health care professionals.
• Maintains current clinical competency per department guidelines.
