Job summary

Bear Valley Community Healthcare District is seeking a Physical Therapist to evaluate, plan, direct and administer medically prescribed therapy in hospital and outpatient settings. You will restore function and help patients reach maximum performance while collaborating with PTAs and providing education to patients and families.

Responsibilities include developing care plans, documenting treatment, and participating in department quality and safety programs.

Qualifications

• Graduate of an American Physical Therapy Association accredited training program.
• Current California Physical Therapy license.
• BLS certification required.

Responsibilities

• Provides patient/client care and assessment per established guidelines.
• Maintains accurate documentation of treatment, reports and charges.
• Develops individualized care plans and evaluates treatment effectiveness.
• Educates patients and families, and trains caregivers for post-discharge management.
• Assists with staff orientation and acts as supervisor in absence of the assigned supervisor.
• Participates in Performance Improvement, Infection Control, Safety and Education programs.
