Job summary

Sweetwater Care in Houston is seeking a licensed Physical Therapist to consult with patients, assess movement dysfunction, and review medical histories and referrals to shape individualized rehabilitation plans.

You will supervise PT assistants, document progress and treatment minutes, apply evidence-based practices, communicate with the health team, and ensure regulatory compliance while delivering compassionate care to patients and families.

Qualifications

• Active State Licensure.
• Able to supervise and support physical therapist assistants and students.
• Documents regulatory and clinical information in electronic records per company policy.

Responsibilities

• Consult with patients to learn about physical condition and symptoms; review medical history and referrals.
• Diagnose movement dysfunction by observing and listening to patients.
• Develop individualized treatment plans.
• Coach patients and teach therapeutic massage to ease pain and promote healing.
• Maintain patient records and track treatment goals.
• Use standardized assessments and evidence-based practice to support interventions.
• Assess progress and adjust treatments in the clinical record per regulatory requirements.
• Evaluate treatment outcomes and modify services based on outcomes and referrals.
• Provide consultation and counseling to patients, families, caregivers and other providers.
• Communicate with supervisor and health team about patient progress, barriers and treatment plans.
• Provide comprehensive discharge summaries per regulatory requirements.
• Document regulatory and clinical information in electronic systems and maintain patient records; record treatment minutes.
• Participate in required trainings, in-services and conferences.
• Supervise and support PT assistants, rehab aides and students.
• Adhere to company policies, procedures, and code of conduct.
• Active State Licensure.
