Job summary

The Atlanta VA Health Care System is seeking a Clinical Pharmacy Specialist (CPS) or Clinical Pharmacist Practitioner (CPP) in primary care to provide expert chronic disease state medication management. Positions are at the Atlanta VA Medical Center and affiliated Community Based Outpatient Clinics (CBOCs).

Applicants may be referred if pending education/licensure, and grandfathering provisions may apply for current VA employees.

Qualifications

• Bachelor of Pharmacy or Doctor of Pharmacy degree from ACPE-accredited school.
• Full, current and unrestricted pharmacist license in US/DC/PR.
• English language proficiency in speaking and writing.

Responsibilities

• Design, implement, monitor therapeutic plans for medications.
• Coordinate with providers and interdisciplinary teams.
• Perform physical assessments and order lab tests to evaluate therapy.
• Budgetary involvement and act as mid-level provider.
