Job summary

Nemours Children's Health is seeking a Clinical Pharmacy Specialist in General Pediatrics and Solid Organ Transplant to join our pharmacy team in Wilmington, Delaware. The role combines hospital and clinic-based pharmacotherapy, with a focus on optimizing drug therapy for pediatric patients.

The position requires a PharmD with pediatric hospital experience, licensure in DE/NJ/PA, and active board certification or commitment to obtain within 12 months; BLS/PALS required, ACLS preferred.

Qualifications

  • PharmD required.
  • PGY-1 residency or 5 years pediatric hospital experience required.
  • Board certification in specialties applicable to the position (BCPPS, BCIDP, BCNSP, BCOP, etc.) required; must obtain within 12 months if not certified.
  • Active Pharmacist License (or legally eligible) in Delaware, New Jersey & Pennsylvania required.
  • BLS and PALS required on hire; ACLS preferred.

  • Consults and educates medical staff on medications, Drug protocols and adverse interactions.

  • Supports education and training of pharmacists, residents, and students; coordinates with multidisciplinary committees.

Job description

Nemours Children's Health is seeking a Clinical Pharmacy Specialist (General Pediatrics/Solid Organ Transplant) to join our pharmacy team in Wilmington, Delaware.

Essential Functions:
  • Evaluates medication orders for accuracy using sound principles, accounting for pharmacology of the drug, pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination with responsibility for establishing and continually improving the delivery of pharmaceutical care to patients within areas of direct responsibility and assisting others in the department with the same.
  • Consults, educates, and works cooperatively with internal customers (e.g., members of the pharmacy staff, medical staff, clinical staff, research staff, nursing staff, and informatics staff) concerning information on medications, therapeutic evaluations, and drug protocols. Develops and provides drug information material. Meets and/or coordinates with medical and/or personnel to resolve problems related to Clinical Pharmacy Services.
  • Provide specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians and practitioners on issues concerning drug therapy in both the hospital and clinic settings, the inherent toxicity of drugs, drug-drug, drug-food interactions & side effects, as well as assisting in the prescribing of appropriate doses.
  • Develops quality improvement projects and participates in the quality improvement and medication use review activities of the department. Collects data; monitors processes; maintain logs, records, and other documentation as assigned.
  • Provides clinical support for multidisciplinary committees, including the Pharmacy and Therapeutics Committee, through the development of materials to support clinical evaluation of drugs, procedures, and processes including the coordination and reporting of adverse drug reaction reporting, pharmacist interventions and other quality initiatives, within areas of direct responsibility. Promotes the use of the NCHD drug formulary by converting non-formulary orders to formulary alternatives when clinically appropriate. Coordinates procurement of non-formulary drugs when necessary.
  • Provides oversight for the accuracy of the online Lexi-comp drug information resource. Promotes the use of the NCHD drug formulary by converting non-formulary orders to formulary when possible and coordinates procurement of non-formulary dugs when necessary.
  • Supports clinical staff pharmacist education and training associated with core competencies and performance review.
  • Participates in policy, procedure, and order set development for the pediatric population.
  • Mentors pharmacists-in-training, including but not limited to Pharmacy Practice Residents, Pharmacy Interns, and School of Pharmacy clerkship students; activities include both practical and didactic experiences.
  • Facilitates orientations and educational programs for the medical staff and residents regarding the appropriate utilization of Pharmacy Services.
  • Able to maintain punctual attendance consistent with NCHD policies, regulatory, and federal, state, and local standards.
  • Achieve certifications, such as Basic Life Support (BLS) Healthcare Provider, Pediatric Advanced Life Support (PALS), and Advanced Cardiovascular Life Support (ACLS), and other required competencies.
Job Requirements
  • Doctor of Pharmacy (PharmD)
  • PharmD with PGY-1 residency or 5 years pediatric hospital experience required.
  • PGY-2 specialized residency training in pediatrics preferred.
  • Hospital Pharmacy experience required. (Pediatric Hospital Pharmacy experiencepreferred).
  • Active Pharmacist License (or legally eligible) in States of Delaware, New Jersey & Pennsylvania required.
  • Board Certification in specialties applicable to the position (BCPPS, BCIDP, BCNSP, BCOP, etc..) is required. Associates hired without active certification must obtain it within 12 months of eligibility.
  • American Heart Association BLS and PALS required upon hire. ACLS preferred.
