US Department of Veterans Affairs at the Tomah VA Medical Center seeks a Formulary Management Specialist to develop, implement, and coordinate clinical pharmacy and medication safety initiatives. The role supports safe medication use and provides educational and administrative services within the Pharmacy department.
The position requires a valid U.S. pharmacist license, ACPE-accredited education (BS Pharmacy/PharmD) or foreign-equivalency with FPGEC, and active English proficiency.
at the Tomah VA Medical Center (VAMC) functions in the Operations section of the Pharmacy service line and will use their knowledge of current developments in field of pharmacy to develop, implement, and coordinate clinical pharmacy and medication safety initiatives to ensure safe appropriate medication use processes for patients while providing clinical, organizational, educational, and administrative services for the pharmacy department.

English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (TRANSCRIPT REQUIRED)

Foreign Education: Graduate of a foreign pharmacy degree program if you are able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). (CERTIFICATION REQUIRED)
Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. (LICENSE REQUIRED)
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
In addition to meeting the Basic Requirements above, the candidate must demonstrate the following below:
Clinical Pharmacy Specialist (Formulary Management Specialist)
Do you possess at least one (1) year of experience equivalent to the next lower grade level (GS-12) or equivalent, which is directly related to the Clinical Pharmacy Specialist (Formulary Management Specialist) position? Develops, upgrades and disseminates acceptable criteria for the use of nonformulary medications; Conducts prospective/retrospective drug use evaluations of high-use, high cost, or high-risk drugs; Assists with managing drug shortages, conversions, and recalls affecting inpatient, outpatient; Accountable for medication reconciliation; Assures the accuracy of drug dispensing of medications; Assists in training new employees; data entry into the computer, screening orders for drug-drug and drug-food interactions, dosing errors, allergies, incompatibilities, contraindications, and formulary availability; Discrepancies are reviewed with the provider and resolved.

Clinical Pharmacy Specialist: The clinical pharmacy specialist (CPS) plays a defined role in budgetary execution and functions to initiate, modify or discontinue medication therapy and as a consultant for intensive medication therapy management services. This includes, but is not limited to, the following: designing, implementing, assessing, monitoring and documenting therapeutic plans utilizing the most effective, least toxic and most economical medication treatments; helping achieve positive patient centric outcomes through direct and indirect interactions with patients, providers, and interdisciplinary teams in assigned areas; performing physical assessments; and ordering laboratory and other tests to help determine efficacy and toxicity of medication therapy.
