Community Health Network seeks an ambulatory anticoagulation clinical pharmacy specialist to support Indianapolis and central Indiana. You will manage DOACs and warfarin through a RN-PharmD hybrid service, provide staff education, develop guidelines, and participate in performance improvement.
You will mentor students and precept residents, collaborating closely with physicians to advance patient care and adherence strategies in a mixed in-person and virtual setting.

As part of a very progressive Ambulatory Care pharmacy team at Community Health Network, you will participate in team-based care under approved protocols where as a pharmacist you may make medication changes, as appropriate. Our ambulatory anticoagulation clinical pharmacy specialist is an integral part of the interprofessional team. As an ambulatory anticoagulation clinical pharmacy specialist, you will manage direct oral anticoagulants and warfarin through our innovative RN-PharmD hybrid anticoagulation service supporting Indianapolis and central Indiana region. Care is delivered both in-person and virtually.
Other aspects of this position include providing staff education, guideline/policy development and implementation, and participation in performance improvement initiatives and practice advancement. You will have opportunities to mentor and teach pharmacy students and pharmacy residents along with serving as a clinical resource for the healthcare team. Your expertise in your field will allow you to advance the level of patient care to achieve the best possible outcomes. This position will provide patient care in the Anticoagulation Clinics and virtually.
