• Each year of Pharmacy Practice Residency counts for 1 year of experience.

Responsibilities

• Plans, directs, and implements clinical pharmacy services to provide high quality patient care by optimizing drug therapy and minimizing adverse effects.
• Provides consultative services to the medical team and drug information to physicians, patients and caregivers.
• Initiates and participates in performance improvement campaigns and continuing education for the medical team.
• Oversees collaborative management of patients with other members of the health care team, including medication reconciliations and discharge teachings.
• Participates in maintaining Joint Commission patient safety goals and regulatory standards of practice.
