Job summary

Qualifications

• Pharm D degree.
• Completion of pharmacy internship and licensure in Michigan.
• One year of hospital pharmacy experience.
• ASHP recognized certification (preferred).

Responsibilities

• Encourage multidisciplinary collaboration to ensure appropriate therapeutic use of clinical drugs and optimal patient outcomes.
• Provides pharmacological information by answering questions and requests of health care professionals; counseling patients on drug therapies.
• Develops hospital staff's pharmacological knowledge by participating in clinical programs; training pharmacy staff, students, interns, externs, residents, and health care professionals.
• Work within appropriate committees to ensure that drugs available are appropriate for the patient populations served.
• Monitors drug interactions and problems, ensuring safety of patients.
