ChristianaCare’s ambulatory pharmacy team is seeking a Clinical Pharmacist to support the Swank Center for Memory Care. The role involves comprehensive medication management, patient and caregiver education, and ongoing monitoring for memory and cognitive disorders.

The successful candidate will collaborate with neurologists and other clinicians to optimize therapy and improve patient outcomes, with opportunities to support additional specialty pharmacy services.

Qualifikationen

• PharmD from ACPE-accredited school required.
• ASHP-accredited PGY1 Pharmacy Residency required or minimum 3 years ambulatory clinical pharmacy experience.
• ASHP-accredited PGY2 Pharmacy Residency preferred.
• Board certification preferred.
• Active licensure as a Pharmacist in Delaware required prior to hire.
• Active licensure in additional states where care is provided or eligible within 2 years of hire.

This position may work remotely up to two (2) days per week.

Join ChristianaCare's growing team of ambulatory clinical pharmacists and play a meaningful role in improving the lives of patients living with cognitive disorders and memory-related conditions. This new Clinical Pharmacist position will serve as the primary pharmacist supporting the Swank Center for Memory Care, working collaboratively with neurologists, advanced practice clinicians, nurses, and other healthcare professionals to optimize medication therapy, improve patient outcomes, and enhance the patient and caregiver experience.

In this role, you will provide comprehensive medication management, patient and caregiver education, medication access support, and ongoing monitoring for patients receiving therapy for memory and cognitive disorders. As part of ChristianaCare's integrated ambulatory pharmacy team, the successful candidate will also have opportunities to support additional specialty and ambulatory clinical pharmacy services based on patient care needs, program growth, and organizational priorities.

This position is ideal for a pharmacist who enjoys building relationships with patients and interdisciplinary teams, thrives in a collaborative and evolving healthcare environment, and is passionate about expanding the role of pharmacists in specialty and ambulatory care settings.

In addition to meaningful clinical work, this position offers flexible hybrid work opportunities, with eligibility to work remotely up to two days per week based on operational and patient care needs.

EDUCATION AND EXPERIENCE REQUIREMENTS

• PharmD from an ACPE-accredited school of pharmacy required
• ASHP-accredited PGY1 Pharmacy Residency required or a minimum of 3 years of relevant ambulatory clinical pharmacy experience
• ASHP-accredited PGY2 Pharmacy Residency preferred
• Board certification preferred
• Active, unrestricted licensure as a Pharmacist in the State of Delaware required prior to hire
• Active, unrestricted licensure as a Pharmacist (or eligible to reciprocate) in additional states where patient care is provided required or obtained within 2 years of hire, as applicable.
• May require intermittent local or regional travel to support specialty clinics, meetings, or multi-site responsibilities.

Hourly Pay Range: $59.43 - $95.08

This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
