Job summary

Qualifications

• Pharmacist degree from an accredited School of Pharmacy.
• PharmD in Pharmacy from an accredited School of Pharmacy.
• Postgraduate Year One (PGY-1) AND Year Two (PGY-2) Pharmacy Residency or equivalent experience.
• Oregon Pharmacist License upon hire.
• Board Certifications from the Board of Pharmacy Specialties within 2 years of hire.
• Two years of Senior Ambulatory Clinical Pharmacist experience within relevant practice setting - OR - Five years of pharmacy practice with at least 2 years in relevant setting in lieu of PGY-1 and PGY-2 residency.

Responsibilities

• Provide collaborative practice services in collaboration with physicians and the healthcare team.
• Utilize registries and reports/tools to identify target populations and optimize therapy.
• Educate the healthcare team and patients in written and verbal formats; participate in teaching and publications.
• Serve as a drug therapy resource to physicians and other health care team members.
• Participate in selection and training of pharmacy learners.
