You will review orders, optimize therapy, counsel patients and families, and collaborate with physicians, nurses, and the pharmacy team to implement protocols and improve outcomes.

Qualifications

• Pharmacy degree required (Bachelor's or PharmD).
• 0–1 year hospital or retail pharmacy experience preferred (entry level).

Responsibilities

• Accurately prepares, verifies, and dispenses medications per prescriptions and regulations.
• Reviews orders and optimizes therapy as needed.
• Participates in safety initiatives and quality improvement projects.
• Counsels patients and families on medication use and adherence.
• Collaborates with physicians and nurses to resolve medication issues.
• Maintains documentation in the electronic health record.
