Job summary

Mercy Hospital is seeking a Clinical Pharmacist to join Mercy Pharmacy. You will collaborate with physicians, nurses, and other health professionals to optimize medication therapy and serve as a drug information resource.

Responsibilities include reviewing orders, counseling, and participating in education and quality initiatives while adhering to Mercy policies. PRN hours are available, supporting flexible coverage across clinical teams.

Qualifications

• PharmD or BS in Pharmacy from an accredited school (required).
• Current pharmacist license in the applicable state (required).
• Two or more years of clinical pharmacy experience preferred; PGY-1 residency preferred.

Responsibilities

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to address clinical needs.
• Develop medication plans to minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications and dosing.

Find your calling at Mercy! Oversees the preparation and dispensing of medication, medication order verification, insures the monitoring of patient medication therapy. Acts as a resource for the provision of medication information for patients, physicians, nurses, and other healthcare professionals. Identifies, recommends, resolves, and prevents medication-related problems through intervention with the physician. Supervises and directs pharmacy support personnel to ensure the timeliness of medication administration. Follows approved policies, procedures, protocols, regulatory standards, in executing job responsibilities.

In this role, you’ll collaborate with physicians, nurses, and other healthcare professionals to ensure safe, effective, and patient‑centered medication therapy. You’ll serve as a trusted resource for drug information, contribute to clinical initiatives, support education efforts, and play a vital role in delivering high‑quality pharmaceutical care. Key Responsibilities

Clinical & Professional Practice

• Review patient records to ensure appropriate medication therapy.
• Assess patient conditions to ensure treatment plans address all clinical needs.
• Develop medication plans that minimize risk and optimize outcomes.
• Consult with prescribers, nurses, and patients regarding medications, dosing, and therapy recommendations.
• Provide guidance on proper drug administration and assess treatment outcomes.
• Serve as a clinical resource for care teams and maintain subspecialty expertise as appropriate.
• Participate in patient assessment and Medication Therapy Services, including recommending appropriate dosing, scheduling, routes, and monitoring for contraindications and interactions.

Collaboration & Education

• Collaborate with healthcare professionals across the care continuum to support optimal patient care.
• Precept and mentor pharmacy students and residents as needed.
• Present in‑services and educational sessions for medical center and clinic staff.
• Serve on committees and contribute to departmental initiatives such as MUEs, policy reviews, and quality improvement projects.

Operational & Safety Responsibilities

• Evaluate medication orders for accuracy and appropriateness.
• Participate in investigational protocols and documentation as needed.
• Support quality assurance efforts by documenting issues and recommending solutions.
• Complete required expiration checks, inspections, and floor‑stock reviews.
• Maintain compliance with regulatory, licensing, and accreditation standards.
• Follow confidentiality and safety policies consistently.

Technology & Systems

• Use pharmacy information systems (e.g., Epic) for order verification and clinical documentation.
• Utilize drug information software such as Micromedex.
• Work with pharmacy automation systems (e.g., Omnicell, McKesson Automation, ScriptPro, Baxa) as required.

Professionalism & Service

• Demonstrate Mercy Signature Service standards in all interactions.
• Maintain positive relationships with healthcare staff and contribute to an environment of respect and collaboration.
• Meet dress code, PPE, hygiene, and professional conduct standards.
• Manage time effectively to meet required deadlines.

Qualifications

Education

• Bachelor of Science in Pharmacy (BS) or Doctor of Pharmacy (PharmD) from an accredited school of pharmacy (required).

Licensure

• Current pharmacist license in the applicable state of practice (required).

Preferred Experience

• Two or more years of clinical pharmacy experience.
• Completion of a PGY‑1 Pharmacy Residency (preferred).

Transforming the Health of Our Communities

Our Mercy health system was founded by the Sisters of Mercy in 1986. But our heritage goes back more than 195 years. It began with an Irish woman named Catherine McAuley, who was determined to help women and children in Dublin experiencing poverty. Though Catherine had a modest upbringing, she received an unexpected inheritance that allowed her to fulfill that dream. In 1827, she opened the first House of Mercy in Dublin, intending to teach skills to women and educate children. Many volunteers came to help. A few years later, Catherine founded the Sisters of Mercy, one of the first religious orders whose Sisters were free to walk among the poor and visit them in their homes. In 1843, the Sisters of Mercy came to the United States. In 1856, they traveled to St. Louis. In the years to come, they would establish hospitals throughout the Midwest. Many of these would join together in 1986 as Mercy.
