Job summary

The University of Chicago Medicine in Hyde Park, IL is seeking a Clinical Pharmacist for Ambulatory/Oncology Infusion Pharmacy (Registry). You will provide safe, effective infusion therapy and support a multidisciplinary team in a high-complexity setting.

You will supervise pharmacy technicians and participate in staff orientation, education and development. Illinois licensure within 60 days and ASHP PGY-1 residency (or 3+ years hospital experience) are preferred qualifications.

Qualifications

• Graduate of an ACPE accredited College of Pharmacy
• Doctor of Pharmacy or other advanced degree highly preferred
• Eligible for licensure as a pharmacist in Illinois (complete licensure within 60 days)
• Completion of an ASHP accredited PGY-1 Pharmacy Residency required or 3+ years hospital staffing experience

Responsibilities

• Comply with JCAHO, ASHP standards and applicable regulations
• Interpret, evaluate, and dispense drug orders considering safety, efficacy, cost
• Identify and resolve drug-related problems and recommend alternatives
• Verify orders entered by pharmacy technicians and enter orders when needed
• Check medications for labeling, preparation, and storage; manage home infusion workflow
• Supervise pharmacy technicians and oversee quality of work
• Contribute to orientation, education and mentoring of staff
• Perform additional duties as needed by senior management

Join University of Chicago Medicine, as a Clinical Pharmacist in Ambulatory/Oncology Infusion Pharmacy, Registry, located in Hyde Park Illinois. In this role as a Clinical Pharmacist, you will assume responsibility and accountability for the provision of safe, effective, and prompt medication therapy intended for infusion. Through various assignments within the department, they provide support for optimal medication therapy to patients with a broad range of disease states. The Infusion Pharmacy is known for providing exceptional assistance when medical needs may be complex and require special knowledge when collaborating with the medical team.

Job Summary

Responsible and accountable for the provision of safe and effective drug therapy and other pharmaceutical care services intended to achieve outcomes related to the cure, diagnosis or prevention of disease, elimination or reduction of patient's symptoms, or arresting or slowing of a disease process, and improve a patient's quality of life in collaboration with a multi-disciplinary team. The pharmacist also assumes the responsibility for pharmacy technicians that work directly under his/her supervision by ensuring the proper preparation of drug products and managing the workflow.

Essential Job Functions

• Maintains a standard of performance that is in compliance with JCAHO, ASHP practice standards/guidelines, federal/state licensing acts, and UCH and Pharmaceutical Services Policies and Procedures


• Interprets, evaluates, and accurately dispenses all drug orders in the patient's best interest as assessed by appropriateness of drug selection, contraindications, dose, side effects, drug interactions, food interactions, duration of therapy, route of administration, and cost


• Identifies, resolves and prevents potential and actual drug-related problems. Recommend rational therapeutic alternatives as needed to prevent future problems


• Accurately checks medication orders that have been entered into the pharmacy computer system by the pharmacy technician: ensuring that patient allergies, drug duplication, and drug interactions do not exist which contraindicate therapy and that the drug dose is correct. Enter orders directly into computer system if necessary


• Checks all medications prior to dispensing for accurate labeling, preparation, and storage. Coordinates all clinical and dispensing workflow that occurs from the home infusion pharmacy


• Supervises the quality/quantity of work and work performance of the pharmacy technician


• Participation in the orientation, education, and mentoring of new staff (i.e. pharmacists, nurses, pharmacy technicians, etc.) as needed


• Performs other duties as requested by senior management

Required Qualifications

• Graduate of an ACPE accredited College of Pharmacy


• Doctor of Pharmacy or other advanced degree highly preferred


• Eligible for licensure as a pharmacist in the state of Illinois (complete licensure process within 60 days of employment).


• Completion of an ASHP accredited PGY-1 Pharmacy Residency required or 3 plus years of hospital staffing experience.

Preferred Qualifications

• Advanced Degree, Master of Science, MBA, or Doctor of Pharmacy highly preferred.


• Clinical pharmacy experience


• Extensive clinical experience and development may be considered in lieu of formal training

The pay range is based on a full-time equivalent (1.0 FTE) and is reflective of current market data, reviewed on an annual basis.
