Qualifications

• PharmD required; Doctorate of Pharmacy strongly preferred.
• Bachelor's degree in Pharmacy required; FPGEC Certification acceptable.
• Pharmacist licensure in NC or ability to obtain within 30 days of start date.

Responsibilities

• Collaborate with physicians and providers to care for clinic patients.
• Develop appropriate treatment plans with providers.
• Manage ongoing medication dosing and patient monitoring.
• Follow institutional medication management goals (e.g., medication evaluations and reconciliation).
• Educate staff on updated guidelines and best practices for this patient population.
• Help manage medication-related toxicities and adverse effects for clinic patients.
