Area of Responsibilities

Completing pharmacokinetics and other medication dosing consults accurately, in a timely manner, and providing follow-up per THS protocol. Completing TPN consults accurately, in a timely manner, and providing follow-up per THS protocol.
Ensuring safe, appropriate, cost-effective medication therapies for patients according to established policies, procedures, and protocols. Monitoring of drug therapy regimens for contraindications, drug-drug and drug-food interactions, allergies, and appropriateness of drug and dose. Reading, extracting, and interpretation of information from patient charts.
Interpretation of medication orders (verbal and written) and transcription to computerized patient medication profiles accurately and in a timely manner. Maintenance of accurate, complete patient medication profiles.
Preparation and dispensing of medication orders per physician request according to established policies, procedures, and protocols in an accurate and timely manner.
Verification of the daily activities and medication-related duties of the pharmacy technician and support personnel staff.
Ability to adapt positively to frequent interruptions and changes in workload and/or work schedule.
Ability to organize and prioritize work assignments.
Ability to work independently with minimal supervision.
Appropriate and timely answering of the telephone and requests at the pharmacy window.
Attendance at Pharmacy Department staff meetings.
