MemorialCare is seeking a Clinical Pharmacist Per Diem to deliver comprehensive ambulatory care services in Fountain Valley and Carson, CA. The pharmacist will manage chronic diseases, educate patients, and collaborate with a multidisciplinary team in the ACTIVE Care Clinic.

Qualifications include PharmD, residency, California license, Advanced Practice licensure eligibility, and strong communication skills.

Qualifications

• Minimum 2 years ambulatory pharmacy experience.
• Comprehensive knowledge of chronic disease management in ambulatory care.
• Effective verbal and written communication skills.
• Proficiency with pharmacology, medical terminology, and disease states for patient management.

Responsibilities

• Provide direct patient care in the ambulatory ACTIVE Care Clinic.
• Manage chronic diseases (diabetes, hypertension, anticoagulation, heart failure) via approved protocols.
• Conduct comprehensive med management, reconciliation, and optimization.
• Monitor outcomes and adjust therapy per guidelines and labs.
• Educate patients on medications, disease states, lifestyle, and adherence.
• Collaborate with physicians, case managers and other health professionals.
• Document clinical interventions in the EHR accurately and timely.
• Identify clinical concerns and escalate to providers when needed.
• Support development and improvement of clinical workflows and protocols.

Position Summary

This position will provide comprehensive clinical pharmacy services within the ambulatory care clinics (ACTIVE Care Clinic), focusing on the management of chronic disease states in collaboration with physicians and multidisciplinary care teams. The clinical pharmacist will manage primary care and specialty patients through evidence-based medication management, patient education, and protocol-driven care. The ideal candidate will demonstrate strong clinical judgment, the ability to work independently with minimal supervision, and effective collaboration within a multidisciplinary setting.

Essential Functions and Responsibilities of the Job

• Work in the multidisciplinary ACTIVE Care Clinic providing direct patient care services
• Manage chronic disease states including diabetes, hypertension, anticoagulation, and heart failure through protocols approved by Best Practice Committee
• Conduct comprehensive medication management, medication reconciliation, and medication optimization
• Monitor patient outcomes and adjust therapy based on clinical guidelines, laboratory values, and patient-specific factors
• Provide patient education and counseling regarding medications, disease states, lifestyle modifications, and adherence strategies
• Collaborate closely with physicians, case managers and other healthcare professionals to optimize patient care
• Document clinical interventions and patient encounters accurately and timely within the electronic health record
• Identify and elevate clinical concerns appropriately to providers and leadership when needed
• Participate in development, implementation, and continuous improvement of clinical workflows and protocols
• Support quality improvement initiatives and organizational goals related to patient outcomes, productivity, and growth
• Maintain current knowledge of evidence-based guidelines and best practices related to ambulatory care pharmacy services
• Possess strong analytic and problem-solving skills

*Placement in the pay range is based on multiple factors including, but not limited to, relevant years of experience and qualifications. Minimum Requirements

Qualifications/Work Experience:

• Minimum 2 years clinical ambulatory pharmacy experience required
• Comprehensive knowledge of management of chronic diseases in the ambulatory setting (including but not limited to diabetes, hypertension, anticoagulation and heart failure
• Effective verbal and written communication skills
• Comprehensive knowledge of pharmacology, medical terminology, pathologies, disease states and related information to medical management plan
• Ability to utilize analytical skills related to collection, analysis and reporting of data
• Basic PC skills including Microsoft Word, PowerPoint and Excel required

Education/Licensure/Certification:

• Doctor of Pharmacy Degree required
• Successful completion of post graduate residency training required
• Current license to practice pharmacy by the California Board of Pharmacy required
• Eligible to obtain Advanced Practice Pharmacist licensure required
• Bilingual preferred
