Job summary

You will enter orders, monitor therapy, and collaborate with prescribers to ensure safe, effective patient care in a hospital pharmacy setting.

This role requires strong communication and computer skills, with the ability to work varied shifts and support oncology, ICU, and other wards as needed.

Qualifications

• Licensed Pharmacist in the State of Ohio.
• Excellent interpersonal, decision-making, facilitation, conflict resolution and investigative skills.
• Excellent listening, verbal and written communication skills.

Responsibilities

• Accurately enters medication orders into the computer system and maintains patient profiles.
• Provides drug and toxicology information to customers to maximize knowledge and safety.
• Calculates caloric/electrolyte content for TPN for correct nutritional dosing.
• Monitors therapy for interactions, allergies, and adverse events to minimize harm.
• Applies pharmacokinetic/pharmacodynamic principles to individualize dosing.
• Contacts prescribers for clarification to avoid delays or errors.
• Provides inservice education on latest treatments and therapies.
• Assists in inventory and ordering to minimize stock outages and maintain appropriate inventory.
• Supports formulary decisions and procurement of quality generics to provide affordable care.
• Maintains medications on patient care areas and reports to the manager.
• Attends nursing unit meetings to share information with management.
• Prepares emergency carts/boxes with in-date medications.
• Responds to Code Blue/Code Trauma to maximize survival and stabilization.
• Adjusts doses for renal/hepatic impairment to avoid toxicity.
• Individualizes dosing for neonatal, pediatric and geriatric patients.
