Job summary

Qualifications

• Bachelor of Science degree in pharmacy is required.
• Experience with unit dose and IV admixtures is desirable.

Responsibilities

• Maintains competency in drug therapy for patients of all ages.
• Provides pharmaceutical care by assisting physicians in reviewing medication histories, educating patients on drug therapy, developing therapeutic plans and monitoring outcomes.
• Monitors drug therapy to evaluate appropriateness of use, dosing, interactions and duplications.
• Documents clinical interventions and cost avoidance as part of the department program.
• Insures timely and accurate dispensing of drugs and solutions from the pharmacy area.

Education

Bachelor of Science in Pharmacy

Pharm.D. Degree or ASHP residency
