Qualifications

• Comprehensive clinical review of pharmacotherapy for patients, including drug interactions and adherence.
• Counsels patients on new therapies and ensures education aligns with management models.
• Follows REMS SOPs and coordinates care with clinicians and caregivers.
• Acts as a liaison between patients, caregivers, and clinical teams to ensure quality care.

Responsibilities

• Clinical review of pharmacotherapy for each patient.
• Document results in patient charts for follow-up.
• Counsel patients on new drug therapy and ensure education.
• Coordinate patient care with clinicians and care teams.
• Provide on-call coverage as directed by Pharmacist-in-Charge.
• Oversee medication dispensing accuracy and workflow.
