The Centers in Cleveland seeks a licensed pharmacist to support pharmacy policy and provide clinical drug information to patients, nurses, and physicians. This role emphasizes safe and effective medication use within a collaborative care setting.

The pharmacist will review orders, dispense to outpatients, counsel patients, supervise drug packaging, and ensure 340b compliance. Ongoing education and maintaining high professional standards are expected.

Qualifications

• Licensed pharmacist in Ohio (state-licensed)
• Vaccination certification required
• Strong communication and ability to work in interdisciplinary teams

Responsabilités

• Review physicians' orders for appropriateness and ensure correct dosing and interactions.
• Dispense prescriptions to outpatients per policy; counsel patients and maintain patient profiles.
• Supervise pre-packing of drugs and control procedures.
• Provide dosing, compatibilities, and formulary substitutes to nurses, physicians and other health care staff.
• Maintain compliance with 340b pharmacy regulations.
• Verify work by support staff, including data entry and unit-dose distribution.
• Maintain up-to-date knowledge through continuing education (NSCE).
• Ensure high quality professional services are maintained.
• Perform other related duties as assigned.
