Job summary

AdventHealth in Palm Coast, FL is seeking a Pharmacist to educate patients and care teams on safe medication use, manage therapy, and participate in rounds. The role includes supervising technicians, ensuring accurate dispensing, and contributing to safety and quality initiatives.

Licensed Pharmacist (RPH) with PharmD is required; 1+ hospital experience is strongly preferred. This is a full-time night shift position in the United States.

Qualifications

• Pharmacists must be licensed and have a Doctor of Pharmacy degree (PharmD).
• Hospital experience strongly preferred.
• Ability to educate patients, families, and healthcare teams on safe medication use.

Responsibilities

• Educate and counsel patients, families, and healthcare teams on safe medication use.
• Provide safe, timely, and cost-effective medication therapy; participate in stewardship and surveillance.
• Engage in direct patient care and rounds; ensure smooth transitions of care.
• Supervise and mentor pharmacy technicians, interns, and residents.
• Identify and report medication safety events and errors.
• Demonstrate proficiency with EHRs and pharmacy informatics to optimize care.
• Contribute to practice advancement through research and committee participation.
• Perform all pharmacist duties across central, satellite, and IV areas.
• Review orders and patient profiles to prevent drug-related problems; adjust dosing.

Job Description:

• Educate and counsel patients, families, and healthcare team members on safe and effective medication use.
• Provide safe, timely, and cost-effective medication therapy; actively participate in stewardship, surveillance, and restricted-use programs.
• Engage in direct patient care and interdisciplinary rounds, ensuring smooth transitions of care from admission through discharge and follow-up.
• Supervise, mentor, and evaluate pharmacy technicians, interns, students, and residents; support team learning and development.
• Identify, report, and help resolve medication safety events, adverse drug reactions, and errors.
• Demonstrate proficiency with EHRs, pharmacy informatics systems, and medication automation to optimize clinical decision support and documentation.
• Contribute to professional advancement through practice innovation, research, specialty alignment (e.g., critical care, oncology, infectious disease), committee participation, and scholarly activity.
• Perform all pharmacist functions including compounding, dispensing, verification, and documentation of medications across central, satellite, and IV areas.
• Review and monitor medication orders and patient profiles to prevent, identify, and resolve drug-related problems; complete pharmacist consults and perform pharmacokinetic dosing, monitoring, and adjustments.
• Participate in quality improvement initiatives, regulatory compliance, and accreditation activities; track performance metrics (e.g., interventions, error reduction, cost savings).
• Performs other duties as assigned.
