Job summary

UPMC seeks a Full-Time Clinical Pharmacist to support Rx Partners At Home and Hospice/SNF pharmacy operations, providing clinical oversight, medication therapy management, and regulatory guidance for partner facilities.

The role requires weekend/evening/overnight/on-call rotations, collaboration with multidisciplinary teams, engagement in quality improvement initiatives, and education of students, residents, and staff.

Responsibilities

• Lead patient care services, clinical programs, and therapeutic initiatives.
• Collaborate with multidisciplinary teams to optimize outcomes.
• Develop, implement, and evaluate pharmacy programs and stewardship initiatives.
• Monitor medication utilization via data analysis and quality projects.
• Provide direct clinical services: MTM, monitoring, reconciliation, discharge counseling, on-call.
• Identify medication problems and implement safety interventions.
• Serve as clinical resource to physicians, nurses, and staff.
• Support regulatory compliance and oversight of pharmacy technicians.
• Develop policies and best practices for medication management.
• Contribute to education of students, residents, staff.
• Lead initiatives to advance pharmacy practice and organizational goals.
