PharMerica is hiring a full-time Pharmacist in Virginia Beach, VA to join our closed-door pharmacy team serving long-term care and senior living clients.

You will evaluate orders, provide clinical recommendations, maintain resident profiles, ensure lab monitoring, collaborate with clinicians, and educate staff. Benefits include Medical, Dental, Vision insurance, 401(k), and more.

Formación

• Bachelor's Degree in Pharmacy, PharmD preferred.
• One year experience in long term care.
• License/Certification: current and valid pharmacist license in the state of practice.
• Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA).
• Proficiency in electronic medical records and pharmacy information systems.
• Strong clinical judgement and effective communication.

Responsabilidades

• Evaluate physician medication orders for drug appropriateness, dosage, interactions, and administration route.
• Provide clinical recommendations to physicians and facility staff.
• Maintain up-to-date patient/resident profiles including medical history, labs, and current medications.
• Ensure tests for high-risk medications are ordered, reviewed, and communicated.
• Collaborate with physicians, nurses and healthcare professionals to provide clinical support.
• Document communications and interventions in medical records.
• Implement and enforce policies for safe procurement, storage, and disposal of meds.
• Ensure compliance with state and federal regulatory requirements.

Schedule: Rotating Schedule- Monday Friday 8:00am - 5:00pm & 10:00am - 7:00pm w/ rotating weekends (Sat/Sun 8:00am - 3:00pm)

As a Pharmacist you will:

• Evaluate physician medication orders for drug appropriateness, correct dosage, potential drug interactions, and proper route of administration.
• Provide clinical recommendations to physicians and facility staff regarding medication therapy changes and optimizations.
• Maintain up-to-date patient/resident profiles, including medical history, lab results, and current medications (prescription, OTC, and home remedies).
• Ensure laboratory tests required for monitoring high-risk medications are appropriately ordered, reviewed, and communicated to the healthcare team.
• Collaborate with physicians, nurses, and other healthcare professionals to provide clinical and technical support.
• Document all communications and clinical interventions in the patient's medical record, including interactions with families and caregivers.
• Implement and enforce policies and procedures for the safe procurement, storage, administration, and disposal of medications and biologicals.
• Ensure compliance with all state and federal regulatory requirements related to pharmacy practice and medication management.
• Report medication-related errors and adverse drug events to the Pharmacy Manager as required.
• Educate and support facility/agency staff on safe and effective medication handling procedures.
• Perform other pharmacy-related tasks and responsibilities as assigned.

Required:
