Job Profile Summary. This role focuses on providing pharmacy related services in a hospital or retail setting. It is a professional individual contributor role that may direct the work of other professionals or manage processes and programs. The role involves overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typically acquired through advanced education. It is an experienced level role that applies practical knowledge of the job area, works independently with general supervision, and may influence others within the job area through explanation of facts, policies and practices.

Job Overview. Clinical pharmacists work closely with other health practitioners to meet patient needs by reviewing and approving medication orders, monitoring drug therapies, and providing drug information. The position improves patient health by providing pharmaceutical care and serves as a resource to physicians, nurses, and other health professionals. It provides leadership by facilitating the development, implementation, and maintenance of hospital and departmental systems that promote desirable patient outcomes. It also educates students, interns, residents, nurses and patients and may participate in drug therapy research. Supervises and directs support personnel.

Minimum Qualifications

• Bachelor’s degree in Pharmacy.
• Pharmacist License.
• One (1) year of related experience.

Preferred Qualifications

• Doctor of Pharmacy (PharmD).
• Board Certification.
• Four (4) years of related experience. Clinical pharmacy expertise with direct patient care experience required, either through residency training or prior job experience.
• Experience in drug utilization review and analysis.

Duties and Responsibilities

• Directs and coordinates the clinical activities of the assigned pharmacy service area. Responsible for preparing, compounding and dispensing medications to patients in accordance with state and federal laws.
• Reviews and interprets physician orders for appropriateness, indication, clarity, dose accuracy, drug allergy, potential drug interactions and duplication.
• Contacts and confers with physicians regarding medication orders; interventions with outcomes are documented in the Meditech Clinical Intervention routine.
• Ensures accurate preparation and labeling of medications; supervises support staff during assigned shifts; ensures operation within regulatory requirements.
• Provides accurate, appropriate and timely drug information utilizing pharmacology knowledge and scientific literature.
• May conduct medication history interviews, reconcile therapies and provide drug information to patients and families.
• Anticipates controlled drug needs based on the Controlled Drug Disposition Record.
• Participates in medication education with patients and documents in progress notes; participates in Medication Use Evaluation.
• Performs order entry and uses pharmacist functions of the Pharmacy computer system accurately and efficiently, including allergy and charging information.
• Participates in multidisciplinary team meetings, reviews discharge medication lists, and provides medication optimization consults.
• May conduct home visits to review medications and educate patients on how to take medications.
• Participates in Pharmacy Quality Programs related to People, Clinical excellence, Operational excellence, Financial performance and growth, Safety and quality, and Research.
• Collaborates with other pharmacists to provide operational support, including weekend, holiday, evening, night and on-call coverage as per departmental policies.
• Provides continuing education for clinical staff and serves as a preceptor for students, interns, and residents.
• May participate in development and implementation of treatment guidelines, protocols, and clinical pathways.
• May participate in departmental initiatives related to drug-specific and population-specific care.

Physical Requirements

• Frequent standing and walking, lifting 5-10 lbs.
• May require lifting and carrying light loads, including boxes and equipment; stooping or kneeling.
• Manual dexterity to operate prescription medications and computer keyboard; ability to see medication labels and computer screens.
