Job summary

Valley Vital Care, Inc. seeks a Clinical Pharmacist (Pharmacist II) to manage medication verification, clinical review, patient counseling, and sterile compounding oversight within our infusion and specialty pharmacy services.

The role requires an active pharmacist license, PharmD or BS in Pharmacy, and 2–5 years of relevant practice. You will lead technician activities, ensure regulatory compliance, and collaborate with care teams to optimize therapy and safety.

Qualifications

• Active Pharmacist license in the state of practice.
• PharmD or Bachelor of Science in Pharmacy from an accredited institution.
• Two to five years of pharmacy experience including infusion, specialty, hospital, ambulatory, or related clinical practice.
• Demonstrated ability to independently perform clinical medication review and interventions.
• Strong knowledge of state and federal pharmacy regulations.
• Excellent communication and interdisciplinary collaboration skills.

Responsibilities

• Verify medication orders and assess dose appropriateness using patient-specific information.
• Conduct prospective medication reviews, allergy screenings, and drug utilization reviews.
• Counsel patients and caregivers on therapy, administration, storage, side effects, monitoring, and expectations.
• Document clinical interventions and patient care activities accurately.
• Supervise technician compounding and dispensing activities.
• Perform final verification and release of compounded sterile preparations.
• Address questions related to stability, sterility, beyond-use dating, labeling, and administration.
