Job description

Overview

Provide pharmaceutical care services to the patients of the Duke University Health System including oversight, monitoring, preparation, dispensing and documentation of patient medication therapy. Deliver drug information, advise clinicians, and support continuing education for pharmacy, patient care services, medical staff, and students while ensuring compliance with federal, state and local regulations.

Duties and Responsibilities
  • Review and monitor patient medication therapy and orders for safety and efficacy; advise the patient care team and document interventions.
  • Prepare, label and dispense medications per physician prescriptions; maintain compliance with laws and required records.
  • Maintain dispensing records and manage charge/credit systems for medications.
  • Provide drug information and education to caregivers, patients and students; oversee all aspects of the medication process and present data and recommendations.
  • Ensure appropriate medications are available, properly procured, prepared, stored and maintained.
  • Supervise personnel involved in drug preparation, maintenance and dispensing.
  • Support investigational medications, including information on preparation, labeling and availability; maintain required documentation.
  • Advise physicians on drug therapy, toxicity, side effects and appropriate dosing.
  • Contribute to education of students and pharmacy residents.
  • Maintain liaison with medical and nursing staff; provide timely information on supplies, usage, regulations and standards.
  • Conduct medication histories, assess compliance, suggest modifications, instruct patients and participate in multidisciplinary rounds.
  • Individualize medication regimens using sound principles, considering pharmacokinetic and pharmacodynamic variations.
  • Conduct or co‑conduct research to impact quality care or evaluate pharmacoeconomic outcomes.
  • Perform related duties incidental to the described work.
Knowledge, Skills and Abilities
  • Effective written and oral communication.
  • Extensive knowledge of drug products, medical therapeutics, pharmacokinetics and disease states; ability to work with information systems and technical equipment.
  • Ability to establish and maintain working relationships with healthcare team members.
Minimum Qualifications

Education: Doctor of Pharmacy Degree or BS Pharmacy Degree

Experience: With a BS Pharmacy degree, two years of practice, acceptable board certification, or completion of an accredited residency program with demonstrated positive patient outcomes; no experience required for Doctor of Pharmacy degree.

Licensure: Licensed Pharmacist or eligible to practice pharmacy in North Carolina
