Job summary

LiveWell Homecare Agency, LLC is seeking a licensed pharmacist to deliver comprehensive pharmaceutical care in a community setting. The role includes dispensing prescriptions, counseling patients, immunization administration, and ensuring adherence to regulatory standards.

Candidates should hold a PharmD or B.Pharm with active license, immunization training, and strong communication, leadership, and organizational skills to uphold patient-centered care and quality service.

Qualifications

• Must hold a PharmD or Bachelor of Pharmacy with an active license.
• Completion of ACPE Accredited Immunization training is required.
• Strong knowledge of pharmacy operations, regulatory requirements, and patient care standards.
• Excellent communication, leadership, and organizational skills.
• Commitment to ethical practices, compliance, and customer satisfaction.

Responsibilities

• Process and accurately dispense prescriptions while maintaining confidentiality and proper documentation.
• Deliver expert advice to support customer health needs and develop actionable plans for service improvement.
• Build relationships with stakeholders, identify business needs, monitor progress, and adapt to organizational changes.
• Model and enforce customer service standards, ensuring issues are resolved effectively.
• Ensure compliance with company policies, regulatory guidelines, and safety protocols through monitoring, audits, and implementing improvement measures.
• Maintain patient safety and quality care by counseling, documenting, and analyzing areas for improvement.
• Promote a culture of integrity, respect, and teamwork, fostering an inclusive environment and mentoring others.
• Support continuous learning, innovation, and excellence in service delivery.
