Job summary

Heart Hospital of Austin is seeking a Pharmacist to monitor, evaluate and optimize pharmaceutical care for patients. You will interpret medication orders, dispense medications and monitor therapy for safety and effectiveness.

Qualifications

• Pharmacist license with Texas State Board of Pharmacy (TSBP) required.
• Pharmacist degree: PharmD or BS in Pharmacy.
• Hospital experience preferred.

Responsibilities

• Interpret medication orders and transcribe to computer system.
• Accurately provide medication therapy and dispense pharmaceuticals.
• Monitor drug therapy regimens for interactions and dosing.
• Suggest cost-effective therapeutic alternatives to medical staff.
• Sustain formulary and promote rational drug therapy selection.
• Assist with clinical services such as drug consults and anticoagulant monitoring.
• Complete notification reports regarding medication errors and adverse drug reactions.

Education

Bachelor of Science in Pharmacy or Doctor of Pharmacy (PharmD)
