Option Care Health in Santa Fe Springs, CA is seeking a Clinical Pharmacist – PT, PD, CA to join our home infusion team. This entry-level role blends patient care with medication management in an alternate site setting.

You will collaborate with Nursing, design patient-specific pharmacotherapy plans, monitor progress, and educate patients and caregivers on safe medication use in accordance with policy and regulatory standards.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license as granted by the State Board of Pharmacy.
• Experience in home care or sterile IV admixture preferred.

Responsibilities

• Provide safe and appropriate pharmacy services in home infusion settings.
• Educate patients, caregivers and staff on medication use and safety.
