The Clinical Pharmacist role is an entry-level position in clinical pharmacy within the home care setting. You will coordinate patient services with nursing, educate patients and caregivers, and monitor progress toward medication-use goals as part

Formación

• Must be a graduate of an accredited School of Pharmacy and hold an active Pharmacist license.
• Experience with customer service in healthcare settings is required.
• Familiarity with sterile IV admixture procedures or related MTM knowledge is preferred.

Responsabilidades

• Assess patient suitability for home care and collect necessary information.
• Design and monitor patient-specific pharmacy care plans for home care.
• Modify care plans based on monitoring data and lab values.
• Provide medication-use education to staff, patients, and caregivers.
• Ensure continuity of pharmaceutical care across home and other settings.
