DMC Sinai-Grace Hospital is DMC’s largest hospital, offering a comprehensive heart center, cancer care, gerontology, emergency medicine, obstetrics/gynecology and cosmetic services. Sinai-Grace’s joint replacement program features a revolutionary minimally invasive knee and hip replacement surgery that attracts patients from all over the country. Sinai-Grace operates more than 21 outpatient care sites and ambulatory surgery centers throughout Wayne and Oakland Counties and is one of 10 hospitals in the nation to be awarded a Robert Wood Johnson Foundation grant to help set the standards of cardiac care for hospitals and physicians throughout the nation.

Job Summary

Under limited supervision, the pharmacist provides clinical pharmacy services including primary care services, consultative services, and drug information services to patients and healthcare providers to deliver high‑quality care. The pharmacist reviews prescriptions as written by authorized prescribers to ensure safety and appropriateness for each patient, contacting prescribers as necessary to clarify vague or illegible orders. The pharmacist fills prescriptions as ordered, dispensing or compounding medication according to established procedures. The pharmacist interviews patients for medication history and provides counseling on current drug therapy, identifies patients requiring further education, and supplies educational materials. The pharmacist collaborates with other healthcare providers to coordinate patient education.

The pharmacist assigns, educates, trains, and supervises support personnel (technicians, residents, interns, clerks and students) in medication preparation and related duties, ensuring accuracy and compliance with established policies and procedures. The pharmacist maintains an inventory of medication and pharmacy supplies and related records, ensuring compliance with environmental and regulatory agency procedures. The pharmacist performs patient care area inspections to guarantee proper drug availability, storage and dating.

The pharmacist provides clinical pharmacy services in areas such as therapeutic drug monitoring, anticoagulation service, nutritional support, investigational drug services, hematology/oncology pharmacy, medication use evaluation, pain management service and adverse drug reaction reporting, among others. The pharmacist adheres to system, hospital and departmental policies, procedures, and objectives.

Qualifications

• Bachelor of Science in Pharmacy or Doctorate of Pharmacy from an accredited College of Pharmacy, or completion of foreign equivalency examination.
• Registered Pharmacist and Controlled Substance licensure in the State of Michigan obtained within three months of entry into the job.
