Summary

DHR Health – US:TX:Edinburg Clinical Pharmacists fill orders for parenteral and enteral medications, monitor patient drug therapy, and provide drug information. They supervise and direct support personnel, and may assume the duties of a Pharmacy Director when necessary.

Position Education / Qualifications

• BS Pharmacy degree or Pharm.D. (or equivalent) required
• License to practice pharmacy in the state of Texas required

Responsibilities

• Review prescriptions to ensure accuracy, determine needed ingredients, and evaluate suitability
• Provide information and advice regarding drug interactions, side effects, dosage, and proper medication storage
• Assess the identity, strength, and purity of medications
• Maintain records, including pharmacy files, patient profiles, charge system files, inventory, control records for radioactive nuclei, and registries of poisons, narcotics, and controlled drugs
• Compound and dispense medications as prescribed, or oversee these activities, calculating, weighing, measuring, and mixing ingredients
• Plan, implement, and maintain procedures for mixing, packaging, and labeling pharmaceuticals to ensure quality, security, and proper disposal
• Teach pharmacy students/ interns in preparation for graduation or licensure
• Advise customers on medication selection, medical equipment, and health‑care supplies
• Provide specialized services to help patients manage chronic conditions such as diabetes, asthma, smoking cessation, or high blood pressure
• Collaborate with other health‑care professionals to plan, monitor, review, and evaluate the quality and effectiveness of drug regimens, providing advice on drug applications and characteristics
• Monitor drug therapy regimens for contraindications, therapeutic duplications, drug‑drug interactions, drug‑food interactions, drug‑disease interactions, allergies, and appropriateness of drug dose
• Detect, assess, and report suspected adverse drug reactions
• Provide clinical consultation and clarification to practitioners
• Document all clinical activities and interventions accurately and completely
• Assist with pharmacokinetic consult service and dosing per organizational protocol
• Suggest cost‑effective therapeutic alternatives to medical staff as needed
• Make appropriate dosing recommendations
• Review or interpret culture and susceptibility data for antibiotic appropriateness and recommend changes as needed
• Recommend conversion of intravenous to oral therapy when appropriate
• Facilitate the identification, resolution, implementation, and communication of cost‑effective medication use issues in collaboration with the medical staff
• Maintain pharmacy equipment and areas clean, neat, and well‑organized
• Maintain efficient filing systems, entering medication orders into the computer, generating medication fill lists, and generating reports as needed
• Issue controlled substances and maintain required records
• Assure accurate control and documentation of controlled substances, perform scheduled inventories, and maintain records as required by law and pharmacy policies and procedures
• Utilize appropriate information resources to answer drug information requests and provide accurate, adequate, and timely drug information to the professional staff
• Sustain formulary by minimizing non‑formulary procurements, utilizing therapeutic substitution protocols, and promoting rational drug therapy selection
• Assist in performing physical inventories, rotating stock to ensure use before expiration, maintaining “borrow/lend” records, documenting all recalled drugs, and managing support personnel
• Perform essential duties of the pharmacy director in their absence, handle essential documentation, supervise and direct pharmacy support personnel, and ensure pharmacy services are provided promptly
• Compensate, prepare intravenous, parenteral nutrition, chemotherapy, and other sterile products accurately, read and interpret medication orders, check orders prepared by support personnel, receive verbal and written medication orders from licensed practitioners, screen orders for completeness, accuracy, and clinical appropriateness, check medication labels, check unit‑dose medication carts, supervise the intravenous admixture preparation area, obtain medication histories from patients, caregivers, and providers, maintain accurate complete patient medication profiles, reconcile patient profiles with medication administration records, transcribe orders into computerized patient medication profiles, maintain outpatient/employees prescription profiles and records, participate in medication use review activities, collect and analyze medication use data, review use of targeted medications for compliance with pharmacoeconomic programs, and check prepackaged medications and records
