Job summary

Community Health Systems is seeking a Pharmacist to ensure safe and effective medication use, provide patient education, and support clinical care within the healthcare team. The role focuses on medication safety, adherence, regulatory compliance, and participation in quality improvement initiatives to enhance patient outcomes and care delivery.

You will review orders, counsel patients, monitor therapy, manage inventory, and document interventions in the EHR, contributing to a culture of safety

Qualifications

• Bachelor's Degree in Pharmacy or Doctor of Pharmacy (PharmD) required.
• 0-1 years of hospital or retail pharmacy experience required.
• Active pharmacist license (RPH) in state of employment required.
• Board Certified Pharmacotherapy Specialist (BCPS) or other BPS specialist preferred.

Responsibilities

• Prepare, verify, and dispense medications per orders and regulations.
• Review orders for appropriateness and optimize therapy with recommendations.
• Counsel patients on proper use, side effects, and adherence strategies.
• Collaborate with physicians, nurses, and healthcare team to resolve issues and improve outcomes.
• Participate in safety initiatives, audits, and quality improvement projects.
• Document clinical interventions in the EHR and maintain patient records.
• Manage pharmacy inventory, stock levels, and cost-saving opportunities.
• Stay updated with pharmacotherapy advances and participate in ongoing education.
