Job summary

SIHB in Seattle, WA seeks a pharmacist to perform clinical pharmacy services within a multidisciplinary care team. You will coordinate care with relevant departments and ensure compliance with legal, regulatory, and funder requirements.

The role reports to the Pharmacy Director; you may manage a care team and contribute to program development and quality improvement efforts.

Qualifications

• Experience or willingness to serve AI/AN communities and other underserved groups.
• Board Certification or willingness to obtain within first year of employment.
• Completion of an accredited residency or relevant clinical experience.
• Valid license as a registered pharmacist in Washington state.
• Preferred: experience in clinical settings or prescriptive authority, familiarity with AI/AN health issues, and grants management.

Responsibilities

• Perform comprehensive disease state management with education, counseling, and active med management.
• Serve as an educational resource to medical and pharmacy staff for primary care pharmacy and disease states.
• Assist in developing guidelines for clinical pharmacy services.
• Prepare data, reports, and proposals for funding sources.
• Ensure compliance with laws, regulations, and grant requirements.
• Staff SIHB pharmacy operations (refill auth and dispensing) per FTE allocation.
• Participate in internal quality improvement teams and drive initiatives.

Position Summary: Perform clinical pharmacy services that benefit the patients and care providers at SIHB. Work with a multidisciplinary care team by providing clinical pharmacy services and program development. Coordinate care efforts between the clinical team(s) and relevant SIHB departments. Ensure compliance with all legal, regulatory, and funder requirements.

Job Responsibilities

• Perform comprehensive disease state management services including education, counseling, and active management of medications and disease states.
• Act as an educational and consultation resource to medical staff, pharmacy staff, and medical team members for primary care pharmacy and specific assigned disease states.
• Assist in developing guidelines for clinical pharmacy services
• Assist in preparing data, reports, and proposals as required by funding sources.
• Ensure laws, regulations, and grant requirements are met.
• Staff in the SIHB pharmacy (refill auth and dispensing) as assigned according to FTE allocation.
• Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.

Background Qualifications

Required:

Experience or a desire to serve the AI/AN community and other underserved communities

Board Certification or willingness to obtain within first year of employment

Completion of an accredited residency or relevant clinical experience

Valid license as a registered pharmacist in the State of Washington

Preferred

• Experience working in a clinical setting or under a prescriptive authority protocol
• Familiarity with health and social issues facing American Indians/Alaska Native
• Experience managing or collaborating on disease state grants.

Work Environment: Office hours are 7am-6pm Monday through Friday with occasional extra hours for events.
