Job summary

Option Care Health is seeking an entry-level Clinical Pharmacist to provide safe and effective pharmacy services for home-care patients. You will coordinate with Nursing and educate patients, caregivers, and staff on medication use.

Qualifications

Job Summary

The Clinical Pharmacist is a licensed professional responsible for providing safe and appropriate pharmacy services to Option Care patients in accordance with the organization's policies, professional standards, and applicable regulatory entities. The pharmacist coordinates patient services with the Nursing Department, monitors progress toward goals, and educates patients, caregivers, staff, and other professionals about medication use.

Job Responsibilities

• Determines the suitability of individual patients for home care.
• Collects and organizes all patient‑specific information needed to assess suitability.
• Assesses patients’ suitability for home care per Option Care policies, including medical condition, prescribed medication therapy, and prognosis with defined outcome goals.
• Designs, recommends, monitors, and evaluates patient‑specific pharmacy care plans for the pharmacotherapy of home‑care patients.
• Recommends suitable modifications in prescribed medication therapy or monitoring parameters for a home‑care patient when appropriate.
• Prevents, detects, and resolves medication‑related problems by collecting and organizing patient‑specific information and making appropriate medication therapy recommendations.
• Identifies appropriate monitoring parameters for each patient.
• Completes nutritional assessment and makes appropriate recommendations.
• Modifies pharmacy care plans based on evaluation of monitoring data and other pertinent information.
• Demonstrates competence in pharmacokinetic principles and laboratory value assessment.
• Provides medication‑use education to Option Care staff, other professionals, patients, and/or caregivers using effective techniques.
• Ensures continuity of pharmaceutical care across all patient‑care settings and uses a systematic procedure to communicate pertinent information.
• Prepares and dispenses medications following Option Care policies, calibrates equipment, and ensures correct concentration, stability, compatibility, and storage.
• Adheres to safety and quality assurance practices, including accurate labeling and final medication inspection.
• Complies with state/federal laws, USP standards, DEA controlled substance regulations, and perpetual inventory system policies for schedule II narcotics and other medications.
• Manages, maintains, and troubleshoots medication administration equipment and related equipment for home‑care patients; provides advice to others on use and maintenance.
• Participates in assessing, managing, and reporting medication‑related incidents and equipment errors.
• Accurately completes QAR documents per Option Care policy and monitors discrepancies in inventory systems.
• Responds promptly to drug‑information requests from health‑care providers, patients, and caregivers, formulating responses based on literature analysis.
• Attends educational seminars or CEU opportunities to promote professional competency and meets state‑mandated CEU requirements.
• Manages investigational drug products per established regulations and Option Care policies.
• Utilizes pharmacy support personnel effectively, prioritizes workload, and ensures accuracy of their work.
• Maintains confidentiality of patient and proprietary information, observing legal and ethical guidelines.

Basic Education and/or Experience Requirements

• Graduate of an accredited School of Pharmacy.
• Current, active Pharmacy license granted by the State Board of Pharmacy.
• One of the following:
  • At least one year of experience in sterile IV admixture procedures.
  • Knowledge of non‑sterile compounding, immunizations, and medication therapy management (MTM).
  • Completion of an ASHP accredited Pharmacy Residency Program.
  • Completion of an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy.
  • Equivalent experience approved by the Vice President of Pharmacy and/or Senior Vice President of Clinical Services.

Basic Qualifications

• Experience providing customer service to internal and external customers, meeting quality standards and evaluating customer satisfaction.
• Basic level skill in Microsoft Word (editing, formatting, tables, spell‑check, etc.).
• Basic level skill in Microsoft Excel (workbook creation, formatting, currency, copying, printing, etc.).
• Basic level skill in Microsoft PowerPoint (slide creation, editing, printing, etc.).

Travel Requirements

Willing to travel up to 5% of the time for business purposes within the state.

Preferred Qualifications & Interests (PQIs)

• Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience with an accredited school of pharmacy.
• Experience in home care, hospital, or other clean‑room settings.

Management Requirements

At least one year of indirect management experience of team members, including assistance with development, training, and assignment of work/projects to others.

Salary

The salary range is $117,548.49 – $195,936.65, determined by education, experience, knowledge, skills, and abilities, as well as internal equity and market alignment.
