Mon-Fri 6:30 am – 3:00 pm. Works every third weekend/holiday.

Responsibilities
- In collaboration with physicians and allied health professionals, evaluate drug therapy based on acuity potential for problems, and provide medication‑related care to achieve definite outcomes that impact length of stay, patient satisfaction, cost containment, and satisfaction of other health professionals.
- Identify drug therapy strategies that improve patient outcomes and reduce costs.
- Utilize assessment skills for medically complex elderly to monitor responses to drug therapy.
- Provide information to patients, either directly as a consultation or in preparation of patient teaching aids or computer‑generated warnings.
- Conduct drug utilization reviews to assure quality of drug use and determine if criteria are met in patient cases being reviewed.
- Educate health care professionals and patients in the safe and effective use of medication through informal patient care rounds, staff development programs, or the hospital’s interdisciplinary education program.
- Support and assist interdepartmental projects such as MUE, PandT monographs, newsletter articles, and performance improvement activities.
- Provide drug information to prescribers and other health care professionals; contribute articles to hospital newsletters; assure that pertinent drug information is maintained in the pharmacy.
- Develop criteria and recommend to the Pharmacy & Therapeutics Committee on appropriate medication use or inclusion in the hospital formulary.
- Interpret and evaluate all medication orders, ensuring safe and effective administration with clarification as needed.
- Review physician orders and suggest alternate therapies with clinical intervention documentation.
- Prepare and dispense medications per professional practice guidelines, state laws, and department policies.
- Advise physicians, nurses, and patients about medication contents, therapeutic actions, and possible adverse reactions or interactions.
- Respond to drug information and poison information requests as required.
- Perform computer order entry functions and rotate to all work areas as assigned.
- Actively participate in medication safety programs by identifying opportunities to improve safety, reporting errors, and implementing system‑based changes.
- Monitor prescriptions to prevent adverse reactions, allergies, or drug interactions; counsel physicians, patients, and nurses accordingly.
- Serve as a role model of customer service by responding constructively to hospital staff needs, using respectful communication consistent with professional collaboration.
Qualifications
- Pharm.D. or B.S. in Pharmacy with equivalent relevant experience.
- Current licensure or eligibility to reciprocate in Pennsylvania.
- ASHP‑Accredited Pharmacy Practice Residency or equivalent experience preferred.
- Good standing with Medicare, Medicaid, and other federal and state health insurance programs.
- Equivalent of three years’ experience as a hospital pharmacist.
Credentials

- Previous hospital experience (preferred but not required)
Education or Equivalent Experience
- Bachelor of Arts or Science (Required)
