The position requires a PharmD or B.S. in Pharmacy from an ACPE-accredited school and an active Florida pharmacist license. 1+ year hospital experience is preferred, with PGY1 completion also considered.
A healthcare facility is seeking a Clinical Pharmacist to support safe, effective, and evidence-based medication management within a hospital pharmacy setting. This role provides clinical pharmacy services, collaborates with healthcare teams, and ensures accurate medication preparation, monitoring, and patient care support.
Review, verify, and accurately process medication orders within the pharmacy system.
Compound and dispense medications in accordance with established standards and safety procedures.
Evaluate medication therapy for contraindications, drug interactions, allergies, dosing appropriateness, and overall therapeutic effectiveness.
Support formulary management by promoting appropriate medication selection, therapeutic interchanges, and cost-effective drug utilization.
Provide clinical pharmacy services including drug consultations, pharmacokinetic monitoring, anticoagulation management, pain management support, IV-to-PO conversions, and renal dosing adjustments.
Provide timely medication information and clinical recommendations to healthcare providers, staff, and patients.
Document clinical interventions, medication-related activities, medication errors, and adverse drug reaction notifications.
Collaborate with physicians, nurses, and interdisciplinary teams to optimize patient medication therapy.
B.S. or PharmD from an ACPE accredited School of Pharmacy required.
Active pharmacist license in good standing with the appropriate Florida State Board of Pharmacy.
1+ years of hospital pharmacist experience preferred; candidates with PGY1 completion will also be considered.
