Job summary

Lower Bucks Hospital in Bristol, PA, is seeking a pharmacist to join its hospital-based pharmacy team. The role emphasizes direct patient care, medication counseling, and collaborative rounding with a multidisciplinary team.

You will contribute to diverse services across Medicine, ICU, OB/GYN, Pediatrics, Emergency, Oncology, peri-operative care and outpatient services, while engaging in ongoing pharmacotherapy monitoring and clinical training.

Qualifications

• Current state Registered Pharmacist License.
• B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.
• Completion of PGY-1 residency required.
• PGY-2 residency preferred.
• Board certification in a Board of Pharmacy Specialties clinical specialty preferred.
• BLS Required
• ACLS Preferred
• Experience in hospital pharmacy operations preferred.

Responsibilities

• Coordinate the activities of the pharmacy department to provide high-quality pharmaceutical care.
• Conduct 50%+ of professional time in direct patient care activities including interdisciplinary rounding and patient counseling.
• Participation in patient care committees and ongoing pharmacotherapeutic monitoring in collaboration with health professionals.
• Direct patient care may include observation and evaluation of patient needs and pharmacotherapy adjustments.
• Scope of patient care includes General Medicine/Surgical, ICU, Obstetrics & Gynecology, Pediatrics, Emergency Care, Oncology, Peri-operative, Geriatrics and Outpatient Services.

Responsibilities

Under the supervision of the Director of Pharmacy and clinical coordinator (if applicable), coordinates the activities of the pharmacy department to provide the highest quality of pharmaceutical care to all patients served by the hospital, while ensuring patient safety. Conducts 50% or more of professional time in direct patient care activities including but not limited to interdisciplinary rounding, patient counseling, or participation in patient care committees. Provision of direct patient care encompasses activities such as the direct observation and evaluation of the patient and his/her medication-related needs; the initiation, modification, or discontinuation of patient-specific pharmacotherapy; and the ongoing pharmacotherapeutic monitoring and follow-up of patients in collaboration with other health professionals. The scope of patient care provided may include: General Medicine/Surgical, Intensive Care Services, Obstetrics & Gynecology, Pediatrics, Emergency Care, Oncology, Peri-operative, Geriatrics and Outpatient Services.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

1. Current state Registered Pharmacist License.
2. B.S. degree in Pharmacy required; Doctor of Pharmacy degree preferred.
3. Completion of postgraduate clinical training (PGY-1) required.
4. Postgraduate clinical training PGY-2 preferred.
5. Board certification in a Board of Pharmacy Specialties clinical specialty preferred.
6. BLS Required
7. ACLS Preferred
8. Experience in hospital pharmacy operations preferred.
