Job summary

Access TLC Health Care in Moorpark, CA seeks a Clinical Supervisor (Nursing Supervisor) for its Hospice team. This full-time exempt role coordinates patient care, supervises clinical staff, and supports interdisciplinary collaboration to ensure high-quality hospice services.

The successful candidate will have an active CA RN license, 1+ year nursing experience, and supervisory skills. CPR and a valid driver's license are required.

Responsibilities

• Receive and review home care referrals and conduct visits to assess hospice needs.
• Assign hospice personnel to cases and consult with Medical Director on eligibility.
• Review cases via home visits, conferences, and records for quality of care.
• Guide clinicians to improve performance and service delivery.
• Evaluate diagnoses, prognosis, meds, procedures and clinical course.
• Assist in setting goals and developing individualized care plans.
• Attend and lead interdisciplinary case conferences.
• Perform quarterly record reviews and report findings to leadership.
• Assist in interviewing, hiring recommendations, and orientation of new staff.
• Support planning and evaluation of in-service and continuing education.
• Participate in policy development and organizational goals.
• Promote professional standards and hospice philosophy.
• Engage in QAPI activities.
• Participate in community outreach and public relations.
• Perform other duties as directed by Clinical Director/Director of Patient Care Services.

We are seeking a dedicated and compassionate Clinical Supervisor (Nursing Supervisor) to join our Hospice team in our Moorpark office. This full-time, exempt role is responsible for ensuring that patient care is coordinated, managed, and delivered appropriately while providing supervision and leadership to clinical personnel.

The Clinical Supervisor plays a key role in maintaining high standards of hospice care, supporting interdisciplinary collaboration, and promoting quality outcomes for patients and families.

Essential Job Functions & Responsibilities

• Receive and review case referrals, including conducting home visits as needed to determine hospice/home care needs.
• Assign appropriate hospice personnel to cases and confer with the Medical Director regarding eligibility questions.
• Review and evaluate cases through home visits, conferences, record reviews, and clinician feedback to ensure quality of care.
• Guide and instruct clinicians to promote effective performance and service delivery.
• Review patient diagnoses, prognosis, medications, procedures, and clinical course.
• Assist clinicians in establishing therapeutic goals, setting priorities, and developing individualized plans of care.
• Attend and facilitate interdisciplinary case conferences.
• Conduct quarterly record reviews and communicate findings and recommendations to leadership and staff.
• Assist in screening, interviewing, hiring recommendations, and orientation of new employees.
• Support planning, implementation, and evaluation of in-service and continuing education programs.
• Participate in policy development, revision, and organizational goal setting.
• Promote professional standards, customer service, and hospice philosophy.
• Participate actively in Quality Assessment and Performance Improvement (QAPI) programs.
• Engage in community outreach and public relations activities.
• Perform additional duties as delegated by the Clinical Director/Director of Patient Care Services.

Position Qualifications

• Current Registered Nurse (RN) license in the state.
• Graduate of an accredited school of nursing (BSN preferred).
• Minimum of one (1) year of experience as a professional nurse within the last three (3) years.
• Supervisory or management experience preferred.
• Hospice experience preferred, but not required.
• Ability to work effectively within an interdisciplinary team.
• Strong understanding of hospice philosophy, death and dying, and pain/symptom management principles.
• Excellent observation, nursing judgment, and verbal/written communication skills.
• Current CPR certification.
• Valid driver’s license and insured, reliable automobile in good working condition.
• Compliance with accepted professional standards and practices.

Compensation & Benefits

• Starting salary: $93,600.00 annually
