Job summary

Blue Ridge Care is seeking a Clinical Nursing Manager to oversee nursing services within our PACE program and coordinate day-to-day operations. The role emphasizes leadership, clinical judgment, and collaboration with an interdisciplinary team.

The position requires RN licensure in Virginia, CPR certification, and several years of nursing experience, including elder care. Local travel and on-call responsibilities are expected in a community-based setting.

Qualifications

• Associate Degree in Nursing from an accredited nursing program required.
• Bachelor of Science in Nursing (BSN) preferred.
• Current, unrestricted Registered Nurse (RN) license in the Commonwealth of Virginia required.
• Current CPR certification required.
• Valid driver’s license, proof of automobile insurance, and reliable transportation required.
• Minimum of three years of nursing experience required.
• Minimum of one year of experience working with the frail or elderly population required.

Responsibilities

• Provide direct supervision, guidance, and support to Registered Nurses (RNs), Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Medical Assistants (MAs), and Medication Technicians.
• Maintain an active nursing caseload, with primary responsibility for newly enrolled participants and other participants as assigned.
• Conduct initial, annual, semi-annual, significant-change, and as-needed nursing assessments.
• Develop, implement, and monitor individualized care plans in collaboration with the interdisciplinary team.
• Provide skilled nursing interventions, including assessment, medication management, wound care, injections, chronic disease monitoring, and participant education.
• Participate in IDT meetings and contribute nursing recommendations regarding participant care needs and service planning.
• Assist with onboarding, orientation, training, and ongoing professional development of nursing and CNA staff.
• Participate in competency validation and skills assessments for assigned staff.
• Provide mentoring, coaching, and clinical guidance to support professional growth and development.

Essential Duties and Responsibilities:

• Support compliance with PACE regulations, CMS requirements, DMAS requirements, professional nursing standards, and organizational policies.

Education and Staff Development:

• Assist with onboarding, orientation, training, and ongoing professional development of nursing and CNA staff.

• Participate in competency validation and skills assessments for assigned staff.

• Provide mentoring, coaching, and clinical guidance to support professional growth and development.

On-Call and Coverage Responsibilities:

• Participate in the PACE on-call rotation as assigned.

• Assist with coordination of nursing coverage during evenings, weekends, holidays, call-offs, vacancies, and other staffing needs.

Minimum Requirements:

• Associate Degree in Nursing from an accredited nursing program required.

Work Environment:

• This position is primarily in person and works in healthcare, office, participant-home, assisted living, nursing facility, and community environments.

• The role may involve exposure to moving equipment, medicinal preparations, bodily fluids, communicable diseases, unpleasant odors, and other conditions common to healthcare settings.

• Appropriate infection-control, universal-precaution, and safety practices are required.

• Local travel within the BRIH service area and occasional evening, weekend, and after-hours work are expected.

Physical Demands:

• Physical Requirements - Requires standing, walking, occasional pushing and pulling, and lifting. Must be capable of lifting or carrying up to 50 pounds and pushing or pulling objects exceeding 150 pounds using appropriate equipment, assistance, and body mechanics.

• Visual, Hearing, and Communication Requirements - Requires vision and hearing sufficient to perform assigned duties, with or without reasonable accommodation. Must be able to document services in participant records and communicate effectively with participants, families, caregivers, staff, and providers.
