Job summary

WVU Medicine is seeking an experienced RN to coordinate patient care across the care continuum and to act as a clinical advocate and educator for patients, families, and caregivers. This role emphasizes improving transitions in care and addressing barriers to access.

The ideal candidate will triage referrals quickly, collaborate with multidisciplinary teams, and ensure clear communication between patients and clinicians to optimize outcomes for high‑risk or complex cases.

Qualifications

• Current Registered Nurse license issued by the state where services will be provided or multi-state license via eNLC.
• Obtain Basic Life Support certification within 30 days of hire date.
• Three (3) years of nursing experience.

Responsibilities

• Assesses and coordinates multidisciplinary care across the care continuum with communication, advocacy, and education in a culturally competent manner.
• Clinically triages new patient referrals to ensure timely access to care.
• Collaborates with multidisciplinary care team to expedite sequencing of workup, treatment, survivorship, and end-of-life care.
• Communicates and reinforces clinical information, serving as a conduit between patient and providers to address needs.
• Performs targeted symptom assessment and management for identified patient population.
• Assesses health literacy and educational needs, providing clinical education about diagnosis, treatment, and post-treatment care.
• Assesses and documents patient needs, goals, and preferences and integrates them into treatment and care delivery.
• Bridges gaps in care and addresses barriers to care during transitions.
• Documents all patient encounters in EPIC.
• Assists with staff education, preceptorship, patient education, and participates in process improvement initiatives.
• Participates in all service line team meetings.

Job description

This role is a professional RN with service line specific clinical knowledge who offers individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. Assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to achieve optimal patient outcomes with a focus on high-risk patients or those with complex care needs (including multidisciplinary care). This role has a particular focus on improving transitions in care and adherence to care and functions as a clinical advocate and educator for patients.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).

2. Obtain certification in Basic Life Support within 30 days of hire date.

1. Three (3) years of nursing experience.

PHYSICAL REQUIREMENTS:

1. Frequent walking, standing, stooping, kneeling, reaching, pushing, pulling, lifting, grasping are necessary body movements utilized in performing duties through the work shift.

2. Ability to sit for extended periods of time.

3. Assists with mobility impaired patients.

WORKING ENVIRONMENT:

1. Standard office environment.

2. Multiple different clinic offices and ancillary testing departments.
