Job summary

PAM Health is seeking a Clinical Navigator to be the face of our hospital in the Miami Valley region. You will cultivate referral relationships, manage territory, and assess patients to facilitate referrals and admissions. You’ll work with the Admissions Manager, nursing and other staff to improve service and outcomes.

Qualifications

• Degree in a health-related field from an accredited college or university preferred.
• Current licensure in nursing, respiratory therapy, physical therapy, occupational therapy, or speech and language therapy is required.
• Prior marketing and/or rehabilitation/LTACH experience preferred.

Responsibilities

• Cultivates referral relationships and manages assigned territory.
• Completes patient assessments and makes sales contacts.
• Collaborates with admissions, nursing and internal/external staff to facilitate referrals and admissions.
• Provides feedback for program development and quality improvement.
