Job Description
The Clinical Mental Health Professional utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care with the appropriate care team, assess crisis situations to provide needed resources and referrals for the patient, and provide family and individual treatment as indicated. This position works collaboratively with patients, patients’ significant other, families, healthcare providers, insurers, community resources, and all other involved parties, providing care that aligns with the established care plan in the assigned patient population.
Essential Functions
- Diagnoses and treats various mental, emotional, and behavioral disorders, modifying care plan(s) to reflect changes in patient status.
- Refers patients to other services to improve their well‑being. Referrals may include community services, treatment programs, support groups, psychiatrists, or other mental health providers.
- Works with the multi‑disciplinary team to develop a discharge plan to ensure continuity of care.
- Organizes and prioritizes daily work by assessing new, current, and discharging patient needs in the area(s) of responsibility.
- Performs behavioral health consultation for patients and caregivers, conducts psychosocial assessments, and provides supportive, crisis bereavement and other behavioral health interventions as necessary.
- Educates, communicates, and coordinates with multiple patient care teams and outside agencies to ensure quality care, timely discharge, and to identify and resolve delays and issues. Credentialing through medical staff office and peer review requirements may apply if working in the behavioral health setting.
- May provide support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events.

Minimum Qualifications
- Master’s Degree in Social Work or related field. Education must be obtained from an accredited institution. Degree will be verified.
- Current State Licensure as LCSW, LMFT, CMHC, or Psychologist.
- Excellent written and verbal communication skills.
- Computer skills, including email and word processing.
Preferred Qualifications
- Experience with patients, families, and treatment teams.
Physical Requirements
- Ongoing need to see and read information, labels, assess patient needs, operate monitors, and identify equipment and supplies.
- Frequent verbal communication and quick understanding of spoken information, alarms, and needs, especially during emergency situations.
- Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy, including frequent computer use and typing.
- Ability to lift and utilize full range of movement to transfer patients, bend to retrieve, lift, and carry supplies and equipment, and handle items of varying weights.
- Capacity to walk and assist with transporting/ambulating patients and distributing supplies and equipment, including pushing/pulling gurneys and portable equipment, and navigating crowded rooms.
- Potential requirement to stand in a stationary position for an extended period.

St. Marys Regional Hospital, Grand Junction, Colorado
Hours & Salary
40 scheduled weekly hours. Hourly rate: $35.25 - $54.39, based on experience.
