Job summary

St Joseph Hospice in Lafayette, LA is seeking a Full Time Clinical Liaison RN/LPN to join our team. The clinician will conduct onsite visits, review orders, and coordinate patient transitions to hospice or home health services.

Candidates should hold an active Louisiana RN or LPN license with at least 2 years of experience, possess a professional demeanor and strong communication skills, and have a valid driver's license. Hospice or home health experience is preferred.

• Ensure HIPAA compliance and accurate documentation.

Requirements

Registered Nurse RN Licensed Practical Nurse LPN in the State of Louisiana with a minimum of 2 years experienceHave a professional demeanor with excellent communications skillsBe flexible self driven accountable for results autonomous yet supportive of team effortsHave a valid drivers license proof of valid auto insurance and active clinical license are requiredPrior sales or business development experience preferredHospice or Home Health experience preferredHCS Interactant software experience preferred but not required
