Job summary

AccentCare, Inc. in Newark, Delaware is seeking a Clinical Liaison RN for a full-time, on-site role. You will identify patient needs and facilitate seamless transitions into hospice services, working with physicians, hospital staff and the AccentCare Hospice team.

The position requires RN licensure in the state, hospital and government regulation knowledge, and the ability to build relationships with providers and care teams. Salary is posted as $90,000–$100,000 per year.

Qualifications

• One year of RN experience in local market with knowledge of governmental home health regulations, Medicare requirements, and care plan development techniques.
• Previous medical sales experience preferred.
• Licensed RN currently licensed as a registered nurse in the state of agency operation.
• Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and in good working order.

Responsibilities

• Provide community and provider in-service education in assigned territory.
• Maintain daily communication with Admissions Department and Team Directors.
• Work with Hospice Care Consultants to develop and implement individualized marketing plan.
• Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups.
• Act as a liaison with AccentCare Hospice Medical Director, Team Physicians and Hospice Physicians on behalf of providers and patients/families.
• Make inpatient visits at contracted facilities and communicates with Admissions Department, Team Directors and/or Primary Nurse.
• Provide consultation on hospice and palliative care.

Join AccentCare as a Clinical Liaison VIPC, Inpatient Hospice Admissions RN, where you’ll serve as a trusted resource for patients, families, and healthcare partners — helping ensure those who need hospice care receive timely, compassionate support.

As a Clinical Liaison VIPC, Inpatient Hospice Admissions RN, you will play a key role in identifying patient needs and facilitating seamless transitions into hospice services.

In this role, you’ll need the heart of a nurse and a polished business savvy to interact with physicians and healthcare leadership. When hospital patients transfer to hospice, they may stay in the hospital, but care will transfer to the AccentCare Hospice team.
