Job summary

TeleMate Health is seeking a compassionate Licensed Practical Nurse (LPN) to serve as a Clinical Liaison in the Memphis area. This field-based role focuses on helping patients transition from facilities back to their homes while coordinating TeleMate Health services.

The Clinical Liaison will meet patients at their bedside, educate them about TeleMate Health, and enroll them into services. When not in the field, the role supports the health monitoring team and collaborates with facility and

Qualifications

• Active Tennessee LPN license.
• Experience in SNF, hospital, home health, case management, or care coordination preferred.
• Strong clinical knowledge and nursing judgment required.

Responsibilities

• Visit patients at bedside in facilities and hospitals.
• Educate patients on TeleMate Health services and remote patient monitoring.
• Complete patient enrollment and collect demographic/clinical information.
• Coordinate with facility and hospital staff to ensure smooth transitions home.
• Provide clinical support to TeleMate Health's health monitoring team when not in the field.
• Document patient interactions accurately in the TeleMate Health system.
• Maintain professional relationships with facility and hospital partners.
