Job summary

Eastern Iowa Rehabilitation Hospital in Iowa is seeking a Clinical Liaison to coordinate intake and pre-admission screening, and to perform on-site assessments to determine admission suitability. You will educate patients and families on rehab options, levels of care, and participation expectations.

You will act as the primary contact for external case management, discharge planners, payors, and hospital partners, and conduct educational visits with physicians and other professionals.

Qualifications

• Bachelor's degree preferred; current clinical licensure required (RN, PT, OT, SLP, COTA/L, PTA, LPN) per state requirements.
• Prior marketing/sales experience preferred; prior clinical experience with demonstrated assessment skills and EMR proficiency preferred; formal sales training preferred.

Responsibilities

• Assist and coordinate the intake and pre-admission screening process.
• Perform on-site clinical assessments to determine admission appropriateness.
• Educate patients and families on rehabilitation options, level of care, and participation expectations.
• Serve as the initial contact for external case management, discharge planners, payors, and hospital partners.
• Conduct face-to-face, in-service educational visits with physicians, case managers, and other professionals.
• Inform and educate community decision-makers about hospital services to develop census through outreach.
• Document screenings, decisions, and communications accurately in EMR and tracking systems.
