Job summary

The Care Team Hospice is seeking an experienced Hospice Clinical Liaison (RN/LPN) to strengthen partnerships with hospitals, SNFs, senior living communities, physicians, and other healthcare providers. You will bridge clinical and sales teams, educate partners, and connect patients with the right care at the right time.

In this role you will travel within the assigned territory, maintain a consistent facility presence, and support timely referral and admission processes.

Qualifications

  • Active and unrestricted RN or LPN license required.
  • Two or more years in hospice, palliative care, home health, skilled nursing, geriatric care, or related healthcare experience preferred.
  • Strong knowledge of hospice care, eligibility criteria, and Medicare Hospice Benefit.
  • Excellent communication, presentation, relationship-building, and customer service skills.
  • Prior healthcare liaison, admissions, business development, or sales support experience preferred.

Responsibilities

  • Build and maintain relationships with hospitals, physicians, SNFs, assisted living facilities, and other referral partners.
  • Provide education, consultation, and guidance on hospice eligibility and disease progression.
  • Identify patients who may benefit from hospice and support timely referral and admission coordination.
  • Collaborate with Community Care Liaisons, clinical leadership, admissions, and operations to improve referral-to-admission conversion.
  • Conduct clinical screenings and hospice eligibility assessments within RN scope of practice.
